Agreement for use of CBF Kurdish Display materials

The CBF Kurdish Team has developed display materials in hopes that churches and various groups could use these materials to promote, educate and raise awareness of the Kurdish Unreached People Group in a variety of mission emphasis activities and other similar events. In order to keep this available to all interested persons, we ask that you complete this form and mail it the following address. Contact information for the display is as follows:

By completing and mailing this form, you and the group you represent agrees to the following: 

• You or your organization will be responsible for its transportation and shipping costs to and from the location where the materials will be used.
• You will ensure that all components of the CBF Kurdish Display and materials will remain intact, in good condition with no apparent markings or damage during the time of loan. Also, you agree to pay any costs resulting from necessary repair or replacement that occur during the time of loan.
• You know how to operate and assemble the CBF Kurdish Display or have read the manuals for the display. (This phrase only applies to those who use the tabletop display in addition to the display materials.)
• You will ensure that the CBF Kurdish Display materials and display are returned at the time previously agreed upon with the CBF Kurdish team.
• You will not allow other groups and/or people to use this equipment if they have not made a reservation that includes this signed agreement.
